Types and Uses of Personal Information
When a user visits our website, information that personally identifies the user may be collected and processed by CHK Nutrition employees. Usually, the only information gathered will be knowingly collected, such as the user’s name, business address, mailing address, telephone number, fax number, e-mail address, and professional licensing information. In addition, this information can be collected as a result of the services or the products a user requests information about or purchases from us. Finally, CHK Nutrition can collect or process personally identifiable information, such as a user’s Internet domain or IP address, as part of the normal operation of our website, or during the normal course of a user’s activities on or use of our website. In nearly all cases this data is collected or processed by, or transferred to, CHK Nutrition’s corporate headquarters. When a user visits our website, the user is consenting to the collection, processing, transfer, or use of this information. If a user chooses not to consent to the collection, processing, transfer, or use of this information in this manner, then the user should not visit, or should discontinue visiting, our website.
INFORMATION COLLECTED DURING REGISTRATION PROCESS
By registering with CHK Nutrition, users receive enhanced access to our website, including online ordering. Users who are new to CHK Nutrition must first complete the registration form. During registration, healthcare professionals are required to give their contact information (such as name, address and email address). We request educational background and title only to verify eligibility to place orders on our site, as we sell only to licensed health care professionals. We will not sell, share, or rent this information to others, other than as described in “Information Shared” below. Non-healthcare professionals have general access to our website without the collection of this personal information.
INFORMATION COLLECTED WHEN YOU PLACE AN ORDER
When an order is placed, the user must provide contact information (such as name and shipping address) and financial information (such as credit card number). This information is used for billing purposes and to fill customer’s orders. If we have trouble processing an order, this contact information is used to get in touch with the user. We will not sell, share, or rent this information to others, other than as described in “Information Shared” below.
Personal Information Shared with Third Parties
Without obtaining a user’s express consent, CHK Nutrition will not sell, trade, or disclose a user’s personally identifiable information to any third party, with exception of those parties connected to our patent agreement. Information may be provided to those involved with our patent licensing for the purpose of verifying compliance with our patent agreement. Information will not be used beyond the patent licensing companies. Beyond those companies involved with our patent agreement, we will not sell, trade, or disclose a user’s personally identifiable information to any party; this includes parties who might attempt to market products or services to the user. However, CHK Nutrition can disclose information to a third party who is used to assist carrying out a transaction for a user or providing a service to a user (for example, packaging, sending, or delivering a purchase or information to the user or clearing a credit card transaction). These companies do not retain, share, store or use personally identifiable information for any secondary purposes.
CHK Nutrition might be required to disclose information about the users of our website as a result of a subpoena, a search warrant, or some other legal process authorized by a court of law. In addition, CHK Nutrition might disclose information about the users of our website to a third party who is assisting us in conducting an investigation about unauthorized activity that constitutes a violation of the policies for using our website, or when circumstances dictate the need to protect or defend our rights or property.
Compliance with the California Consumer Privacy Act
It is the policy of CHK Nutrition to comply with the California Consumer Privacy Act. As of January 1, 2020, residents of the State of California have the legal right to request that all personal data be deleted from CHK Nutrition’s database, networks, and server. CHK Nutrition considers personal data to be all personal information enumerated by the California Consumer Privacy Act, such as “information that identifies, relates to, describes, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular consumer or household, such as name, postal address, account name, email address, purchasing history, or other similar identifiers,” and all other personal information defined by the California Consumer Privacy Act.
A California resident who wishes to request that their personal information be deleted from CHK Nutrition’s database, networks, and servers should transmit such a request to email@example.com. Please bear in mind that if you request to have your data deleted, we will need to re-establish your account if you wish to place orders in the future. Only a resident of the State of California, or a person registered with the California Secretary of State authorized to act on a resident’s behalf, can exercise deletion rights under the California Consumer Privacy Act. CHK Nutrition cannot respond to a deletion request if we are unable to verify the identity of the person making the request. A request initiated from a customer account’s email address of record is considered sufficient verification. After receiving a request from a resident of the State of California for deletion of all personal information, CHK Nutrition’s Customer Service Department, Information Technology Department, and Marketing Department will delete all personal information within three business days of receiving the request.
CHK Nutrition takes every precaution to protect your information and regularly implements industry-recommended technological and operational security measures for the purpose of protecting personally identifiable information from unauthorized access, loss, misuse, alteration, or unintentional destruction. Although no security measure can absolutely guarantee against unauthorized uses, CHK Nutrition regularly reviews and updates our website security measures for the purpose of providing appropriate security measures for personally identifiable information.
When users submit sensitive information via the website, your information is protected both online and offline. When sensitive information (such as a credit card number) is required, that information is encrypted and is protected with the best encryption software in the industry – SSL. While on a secure page, such as our order form, the lock icon on the bottom of Web browsers becomes locked, as opposed to unlocked.
While we use SSL encryption to protect sensitive information online, we also do everything in our power to protect user-information offline. All of our users’ information, not just the sensitive information mentioned above, is restricted in our offices. Only employees who need the information to perform a specific job (for example, a customer service representative) are granted access to personally identifiable information. Furthermore, ALL employees are kept up-to-date on our security and privacy practices. Finally, the servers that we store personally identifiable information on are kept in a secure environment protected by the latest firewall technology.
You should take precautions in choosing the information you send to us via email and in keeping your password confidential. You will be solely responsible for all uses of your password, even if such uses were not authorized by you. If you become aware of any unauthorized use of your password, you are responsible for contacting us to request deactivation of your password.

Access and Accuracy
Because CHK Nutrition only wants to maintain accurate information about the users of our website, any user can request to be provided the individual information that has been gathered about the user. Any user can contact CHK Nutrition (see the Contact Information section below), and, upon receiving proper identification information, CHK Nutrition will provide the user with the personally identifiable information that CHK Nutrition maintains about the user. If a user finds a factual inaccuracy, then the user can notify CHK Nutrition and the inaccuracy will be corrected.
We send all new members a welcoming email and to verify your email address (username). At this point, your account is active. You can also set up an account to order online. Registered healthcare professionals can also receive our emails and electronic newsletter which serves to update you on our products, address frequently asked questions and share scientific updates. We may also contact you from time to time with voluntary surveys to gather information to better serve you. Out of respect for the privacy of our users, we present the option to not receive these emails or newsletters. CHK Nutrition will not retain a user’s personally identifiable information longer than is necessary for the purpose it was collected.
– Children under the age of 18 are prohibited from using this site. CHK Nutrition’s sites are not intended or designed to attract children under the age of 18. We do not collect personally identifiable data from any person we know to be under the age of 18.
When a user browses this website, one or more temporary cookies can be placed on the user’s computer to help CHK Nutrition collect information to better understand the user’s interests and to better serve the user. Cookies are used on most major websites. What are “cookies”? Cookies are small bits of information stored by the user’s web browser’s software on the user’s computer’s hard drive or temporarily in the computer’s memory. Using cookies is an industry standard. Cookies enhance a user’s experience and access to information on a website. Cookies enable CHK Nutrition to recognize a user and personalize the user’s experience on the CHK Nutrition website.
In addition to using cookies, CHK Nutrition website can employ “pixel tracking,” a common process used in connection with advertisements and/or internal tracking. Pixel tracking involves the use of pixel tags – an invisible tag that is placed on various pages of a website that tracks a user’s activity. Pixel tags are not visible to users and consist of a few lines of computer code. Pixel tracking measures the effectiveness of advertisements by compiling aggregate and specific usage statistics. CHK Nutrition can access pixel tags to identify the activities and interests that better enable us to match products, services, and offers to the user’s interests and needs. For example, if a user visits the CHK Nutrition website as a result of an advertisement on a third-party website, then the pixel tag will allow the advertiser to track that its advertisement brought the user to a website operated by CHK Nutrition. If a user visits a website or other online system operated by CHK Nutrition, and the user is linked to another website, then CHK Nutrition also is able to determine that the user was sent to and/or interacted with a third-party website. This data is collected for use in marketing and research.
